Meh, I'll go easy mode:

I'm not sure, I haven't really thought about it.
Personally I envision it as a new region bit region with multiple biomes, but that could lead to fatigue; if the map is too vast, why bother? I like the idea of small islands though. If each of them had different biomes and had their land mass randomly generated, it could keep things fresh while giving each player a truly unique experience.

The Zeus/Poseidon and its Chinese variant kind of did that but in a very simplified way for the military.

Ok, seriously. I want a game that's LIKE Zelda, except the main characters odd jobs for people around the kingdom, helping rebuild after the villain was defeated. And eventually turning the kingdom from crumbled, to repaired, then from repaired to flourishing.

I here there's some little cutesy indie game with that premise. But I don't mean a little clickittyy clackity game made for mobile. I mean a full featured adventure game with the style like Breath of the Wild. Except it's all about rebuilding the ruined kingdom, through supporting various industries.

Isekai where you get betrayed and abandoned from everyone and then you slowly level up to take revenge. You can go around and kill everyone or play around with them however you want in a massive world. Want to torture the hero who wronged you? Sure go ahead! A medieval fantasy rpg in the style of Daggerfall set before the last great deluge in what would eventually become North West Europe. The setting would make use of European folk lore heavily as inspiration for various characters, creatures, factions and the like. Fantasy staples like elves and dwarves would make an appearance but with a novel approach, more accurately reflecting what our ancestors believed these creatures to be. With a heavy dose of artistic license of course :)

Terrain generation would use height map data from UK, France, Germany & Scandinavia to create the basic game world geometry.

Something like gary's mod, but with procedurally generated scenario, like no man sky or astroneer, so you can actually drive a long distance and change the scenario with machines and all that.

A Roleplaying Game where there's gameplay that extends beyond combat encounters and charisma checks, to the extent that you can play the game by doing [OTHER THINGS] and have a fulfilling experience.
Basically, what MMOs used to be about before they devolved into raiding and delegating other elements of gameplay to autonomous systems.

In the distant future, planet TDM has hired you as a bounty hunter. Their long arm of the law seeks to capture native outlaws, murders, kidnappers, space pirates, terrorists, and worse, who have gone into hiding on human worlds.

Gameplay involves going on missions where you deploy your team of cute mascots and try to sneak up on other cute mascots in public places to blackbag them, and drag them back to your ship. You can fail if your target notices too soon and gets away, if humans get suspicious, or if civilians are harmed. Between missions, you get to customize your team's equipment, and the interior of your ship. You also have to manage your brig to make sure none escape or riot before you take them back to planet TDM for trial or jail. All of it disconcertingly cutesy.

The goal would be to create a grounded fantasy game world with immersion and verisimilitude in mind. A magic system similar to Tolkiens but more defined and fleshed out. Vampires & werewolves with their own respective lore I could go on.
