Early on in the game I blasted a Powder Ganger in the head at point blank range with a shotgun and he didn't die. This kills the immersion. In Stalker and Metro a head shot downs any human enemies.

It's not really an RPG but I think the way Deus Ex handles "leveling" your character is the best. Guns hurt regardless of your skill with them and the only spongy enemies are cyborgs and robots and shit. Most normal human enemies go down fast.

That can be a multihour endeavour at times and there will still be random CTD's after the fact, unless you're running more or less nothing but viva new vegas or something.
