Same thing happened to me. Early on in the game I blasted a Powder Ganger in the head at point blank range with a shotgun and he didn't die. This kills the immersion. In Stalker and Metro a head shot downs any human enemies.

I'm always turned off by fallout because the shooting feels like shit, plus it feels so dumb that my personal skill with a weapon somehow makes the bullet stronger
Is there some example of an RPG that implements fireweapons in a way that is realistic and satisfying, while still keeping some skill-based system going on?

The irony is that gunplay functioned much more logically in the turn-based rpg games, because your skill determined accuracy and every gun had fixed damage. I have no idea how you could translate those skills to an fps properly, and neither did Bethesda.

If you're aiming for "realism" though, then you have a problem anyway when you start talking about "all headshots should be fatal even close range", because that's literally not the case. The chance of fatality of being shot in the brain close-range is nearly 100%, but the head is not composed of a brain and nothing else, and it's possible to get shot in the brain and not die immediately, to suffer impairation but not death, and in really rare cases to even come out of it relatively unfucked. This is largely due to the fact that bullets tend to break apart on impact, and not just plow through like a cannonball. A shotgun point-blank is probably one of the few cases it actually should be 99% instantly lethal.
Imagine wanting realism, and not implimenting skull graze mechanics. Your game is instantly shit.
