In my experience most of the genuine difficulty came from the overwhelming difficulty of level DIE, however after a while it was much easier to adapt to since everything moved at one speed (fast, but still one speed).
Normal difficulty was worse than Hard for me, because of how the difficulty would fluctuate, change enemy behavior and their attacks. It made it difficult to adjust to the rhythm changing constantly. Yes, I could use the grovel to turn down the difficulty, no that doesn't really solve my gripe with it.
I've played a shitload of it, you do get used to the camera/controls, know where enemies are, use the radar etc. I still never got used to difficulty swaps mid-fight, even when I screwed up and the game slows down the different rhythm would completely fuck me up sometimes.
I came to grips with the game easier by playing on Easy and still doing the arena fights. The game is too unstable on Normal. Hard is easier than Normal for me.
