She realized she was a monster/"ghost" conjured from Alessa's mind just like all the other monsters walking around. I think parts of the "real" Lisa - the drug addicted depressed guilty woman who Alessa never really met - also started spilling into/corrupting the idealized version that Alessa conjured and Harry was interacting with for the whole game. I think the bleeding is not just her turning into a monster like the other hospital staff you encounter but also Lisa regaining her memories of all of the pain, trauma, torment, hopelessness, guilt she felt before she died from overdosing.

I don't know if your OP question is sincere or you just wanted to generate discussion but I feel like Lisa is a character you appreciate more and more with every replay so I highly encourage you to replay the game at some point in the future

I think it + even the bloopers cutscene at the end of the game where she bursts out laughing are very sad because I just find it impossible to imagine a situation where Lisa as a character could ever truly be happy and laugh or smile like that. Even though she is sweet and comforting to Harry she is still portrayed as being very melancholic and tired. Those few images of her smiling are like a peek into what her life was probably like before the drug dependency. A totally different, happier person.

Btw it really is just incredible how well realized these PS1 characters from 1999 are. I feel like I am talking about a real person here lol.

I think having Heather visit Alchemilla instead of Brookhaven in the SH3 Remake could be a really good change for bloober team to make. I played Shattered Memories recently and I think I would really like to see SH3 remake have a version of the "home video" clips that play when you start the game + after the Twist in the ending. A montage of some of the good times Harry and Heather spent together. Because to me SH1 and 3 are all about that bond between father/daughter, protecting your flesh in 1 and then honoring the memory of your blood in 3. Anyway even if there's no video montage we'll probably still get some sweet family photos in the apartment in the remake.
