Something like this has occured for me too before but it was in fallout 3 and my character's head was only a set of floating eyes and teeth. I just kept it, felt like I had won a special opportunity for roleplay or something. Beat the game like that.

Are you using an AMD CPU? I've had similar issues in Mass Effect and other old games that used the old 3DNow instruction set that's no longer supported. Check the PC gaming Wiki or try using the Silent Patch to fix it.
