My rule of thumb is:
If the game has 3 or 4 difficulty levels, always pick the hardest one possible. Mom did not raise a bitch.
If there is 5 or more, or hardest has a clear note that it's intended for 2nd playthrough, pick 2nd hardest.
If game has no difficulty options at all, play through it normally for 2 hours then voulntarily choose one of game mechanics to abandon in order to challenge myself.
Works all the time.
