How come other MMO's haven't been able to top Runescape in quest design? Even single player open world games struggle.

Because
1) Runescape wasn't designed like a proper MMO for most of its lifespan. You can still argue that the multiplayer element still barely matters to its game design, so they put more focus on the solo content.
2) Other MMOs actually have some pretty good quests, it was just more typical that you negros would install add-ons n' shit to blaze past all the dialogue, exploration, and interactions that went into them. People are starting to feel similarly about OSRS's quests now that Quest Helper exists
3) Other MMOs put focus on dungeons to be where more mechanically involved and story-driven encounters could be, which Runescape never really had. Even in their most recent full-on Raids, they still don't match other MMOs

I'm not sure if they still do them but they used to do massive summer events with special mystery questlines. They were very fun to do and the first few people to solve every puzzle got special prizes. Incredibly difficult to solve though and would often require outside game help or insane game knowledge. Some were jumping puzzles which were nice.

SWTOR has better writing, but its gameplay means that you only have 'kill enemy' and 'right click on environment asset' as options. Runescape is a point and click adventure game so every quest feels way more varied even if the stories are way simpler.

You'll have your exceptions, but for every Scarab Lord questline in WoW, there's 500 collect bear ass quests.
