Storywise the outline is solid as is, basically all you'd have to do is extend and expand on it. Maybe something to act as gym leaders, something like old japanese boomers and similarly distinguished members of the community (which may or may not be under Pecharunt's influence) upset at a foreigner like you is questioning and getting in the way of old traditions. Kieran's crash out as the big plot twist/climax of the story, Mochi Mayhem to close it out. It'd work.

Imagine if Kitakami is part of a future region...

It doesn't even have to be the Loyal Three or related to them. Pecharunt is a foil to Pokemon trainers as is. Bring up the question of what makes Pokeballs okay vs a toxic chain to draw the parallel/rivalrly even further.

All clues point to Tohoku. Kitakami has extinct Hissui Pokémon, and an Ursaluna swam from there. In real life, Tohoku is a neighbor of Hokkaido.

The Loyal three are based on Tohoku prefectures. Akita is the birthplace of the Akita Inu breed, Japanese macaques originate in Aomori, and the green pheasant is the symbol of Iwate.

Dipplin is based on the Aomori apple variety, where the largest apple production in Japan is found.

Most importantly, real-life Kitakami is a town in the Tohoku mountains in Iwate Prefecture, where there is an annual Onikenbai festival featuring ogre masks and a mask museum called the Ogre Museum.

That said, if Kitakami doesn't have a league of his own, his best option is to go to Sinnoh in the north or travel to Kanto in the south. Carmine and Kieran are a special case; they're the equivalent of Japanese exchange students studying in America; either they're very good students or their parents have a lot of money.

Carmine was picked for the Area Zero research and Kieran quickly rose through the BB League ranks with just a bit of motivation. They're definitely among the top students.

Okay, Tohoku as an expanded mini-region.

We're starting in Kitakami. The starters are:
A weakling green Shiba Inu puppy, whose final form is a Grass/Fighter Akita Inu judoka.
A pink chick wearing its shell as clothing, whose final form is a Fire/Fairy Kimono-like plumage pheasant.
A blue baby monkey that blows bubbles with its snot, whose final form is a Water/Psychic Japanese macaque with a bubble abacus.

Rival Kieran/Carmine: Depending on your decisions, one will be your friendly rival and the other your jerk rival.

The region expands to three more towns; there are no cities.

Instead of Gym leaders and evil teams, you have to defeat the different family clans in the region.

The new Pokémon will be regional forms of Johto Pokémon mixed with classic Yokai. Maybe 12 new pokemon

As an added bonus, the return of classic characters like Lance and Clair visiting relatives of the Dragon clan to present their incestuous baby.
