Prince of Persia had you take damage or die depending on how high up you fell from. That was in 1989, on the Apple II. Demolition Man wasn't ahead of anything.

Donkey Kong didn't really explore the concept of "damage" though. Jumpman just ceases to exist, you can't really tell if he's damaged or not.
