Something about his eyeball being knocked was so deathly grave to me as a kid. When you die on the last life bottle, and the skull icon flashes on screen before the picture burns away and you get this image, they basically really conveyed that Dan had failed and had died for good.

This used to kind of freak me out how sudden the music comes to stop with the sounds of him/her being impaled. Really well done game over screen.

fear effect 1 had some cool animated game over screens that changed based on your location but I can't find them so have a classic

pic rel and MGS3 allowing for the use of a revival pill during certain "Game Over" screens
any other games like that?

Scarface 2006 has some cool deaths and my favorite is the one which happens once every single playthru even if you expect it :) It's when you have a million in your pocket and decide you could take on a street gang, step out of the car and immediately someone chainsaws your head off and you lose your money lol
