Modded TES, Witcher 3. I also found Final Fantasy X extremely immersive back in the day.

What makes w3 immersive? I find that the story collapses near the end. That was also when the game wasn't fun to play anymore.

NTA, didn't even finish the game, but I found myself absorbed by the environments, music, stories and characters. A somber tone seems to do it for me.

Is that alone. I still claim that w3 has side quests that are incorparated into main quests, at least some of them. Should be the norm and could certainly be done better. Problem is that games aren't made for gamers.

From a technical standpoint I think it's clear that stories in games don't have a high standard. Which is a shame, really.

Gotta define immersive. Modded Skyrim is about the best when it comes to the amount of things you can make it do, including the most basic stuff like using furniture or ingame writing for your RP.
I personally have a preference for fantasy in an urban setting, an itch which I got from Hunter x Hunter and which FFXV rubs somewhat, although the game is so incomplete on a number of levels.

Dark Souls, back when I first played it. The world felt dangerous and I didn't know what to expect, just slowly trying to make my way through the trials. Then the first invasion happened. The decision to not have music at all times was perfect.

FFVI's world of ruin feels like an empty MMO to me.
So it's immersive in the sense that I imagine an alternative reality where the SNES was capable of broadband internet connections and FFVI was an MMO that has been abandoned.

My favorite is Fallout: New Vegas. I put a lot of points on the Survival skill and other non-combat ones so I can live off the range and dumpster diving as much as possible. It's really great to wait until it's dark to craft a bunch of steaks for the road.

There are a lot of things wrong with Skyrim but you can max out comfy to 100 in this game

I'm not saying the quest beats are awesome. It's how some side quests have multiple solutions and some do change the course of the main quest(s). Side quests having an intro and outro is awesome, too. W3 has plenty of downsides, make no mistake.

Well I can say the quests are awesome. There's no gameplay mechanics for alternative solutions like stealth, but the story "makes up" for it.

Get the premise, investigate, read up on the monster, prepare, combat, then some revelation/twist or choice occurs, all resulting in a nice little witcher story.

Now, if we could merge Witcher 3 with an open world sandbox game, with custom characters and gameplay options for warrior/mage/thief archetypes.. We would be approaching a perfect RPG.
