There's also a much later situation where if you have used it during that run, you will have to start over, but you really should know better by then. also a third instance where a character will reward you with a nice stat upgrade if you haven't used it. "forcing ng+" is misleading, you will always need multiple runs to complete the game, it's a roguelike, you need to do multiple dungeon runs just to progress certain variables, in that sense there's a ton of things that will "force ng+" since the true ending needs various criteria
