It's because the whole country has split itself into 2 zones - big city and not big city.
The cities are overpopulated, which causes lower reproductive pressure in a native population, and the rural areas have culturally become unattractive compared to cities for young people, so they move into those cities (making them even more overpopulated and the rural areas even more unappealing as everyone abandons them. This snowballs until nobody want to have children anywhere.
