Merely perceiving the cosmic horror changes the people in the game including the protagonist. Likewise not perceiving it leads to no change.
And keep in mind this doesn't just apply to horror monsters either...it can apply to positive and other stranger things as well.

Sam's room 33 is a dead giveaway that the creator is either a Mason or channeling occult knowledge. The whole visitor actually to a slew of religious prophecies and esoteric predictions about an event that a guy like jaydreamerz on youtube has coined as the "plasma apocalypse" which involves people mutating into monsters, getting possesed, or getting super natural powers. He even talks about how those susceptible to these things will be people look outside or are outside when it happens. Go check his channel out
Personally, it's fucking hate this game especially because I actually believe some of this shit might actually happen and this the dev made sure to showcase the absolute worse scenario with very little upsides. Even the good ending is fucking weird and eery. Satanic piece of shit demonic game.
