She is a rental girlfriend. Rental girlfriends are a thing in Japan. You cannot have a story in a magazine for young boys which the MC pesters one long enough that she relents and dates him, it would send the wrong message. But the author wants to draw a sex scene with her, because that's why young boys buy this magazine for after all. So he compromises and draws a fantasy where a random guy that lives in the MC's head rent free fucks her instead. The guy doesn't know she is a rental, so all's right with the world.

In a good series it would be easily defensible. There's ways to do an MC having guilty/disgusted/anguished intrusive thoughts about his crush fucking another man he's jealous of due to insecurity and not ruining the character or series with it - the entire point of intrusive thoughts is they're involuntary, irrational, unwanted, and not often rooted in actual desires so much as they are self-critique or dark ideation. It doesn't necessarily have to be weird cuckshit where the MC sorta gets off on it while it's mostly used for fanservice and shock value rather than actually examining the characte'rs flaws.

Kanokari is fucking dogshit though so it's just an excuse to draw Chorizo getting railed and stir up some controversy. We didn't really learn anything or see him grow from it; if anything it's a regression to where he was at the start of the series that wasn't properly addressed after because Reiji didn't actually want to work out some sorta character arc from this.
