People get triggered when such shit also happens to the protagonist instead of only to side characters. Nobody ever gives a shit when a nameless side character is raped. That said, I found the anime version of this scene to be even more in-your-face than the manga version, and definitely more sexually charged (for better or for worse). I read the manga ages ago, way before it got an anime, and I remembered it as something 'oh yeah, that happened'. It's shitty, but it's also a shitty world, and it does show that nobody is safe from any consequences, not even the main characters.
Also, if it weren't fucked up, people wouldn't STILL, ages after this shit was even released, go absolutely feral at the slightest mention of it. If it was meant for shock value, that certainly worked, jesus christ.
