There would be a ton of incest doujins with Itachi and snake tentacle rape doujins with Oro if Sasuke was a woman. But I'm not sure Sasuke's coldsteel clan avenger plot would make as much sense if he was a she. A girl Sasuke might want to get revenge against Itachi, but she would go about it completely different than boy Sasuke.
