If you look into it in detail, you'll understand what Ruka's illness is, but in terms of a car, even if you step on the gas when you're going up a slope with your luggage loaded, the engine speed doesn't increase and it slides down the slope.
It starts relatively early in life, but about 70% of cases improve or are cured by adolescence or around the age of 20.
If it hasn't increased by Ruka's age, she would need a pacemaker or surgery, but there's no depiction of that.
In fact, she's excited around Kazuya and she was excited at the pool, so if it's true, it's cured.

It's fine if Miyajima wants to wrap up the story by saying she has arrhythmia, but he'll be criticized for being ignorant.

I think there's a chance that the story will end in a similar state to the instant him and Ruka ended up together (TN: probably referring to a situation similar to the one from chapter 27 happening).

This date arc was a crossroads for fate, and if Mizuhara had accepted Kazuya, Ruka would have been done for.

However, Mizuhara rejected Kazuya because she couldn't do what Ruka can, so the status quo remains.

Ruka will probably find out about the Mizuhara thing, but Kazuya, in his current situation, has no more reasons to push Ruka away anymore.

By continuing his relationship with Ruka, he'll find out she, who didn't talk to him about the subject, is still measuring her heartbeats, and realize that her arrhythmia isn't cured.
