The rule is the “winning” or main girl in a JP harems must the 2nd and rarely the 3rd most attractive of the girls. For a recent example, consider Makeine. The main girl is the most attractive of the “losers” but the “winners” are also a part of the harem, so that makes her actually the 2nd most attractive (Karen being #1).
Also, outside of porn, it’s extremely rare for a harem to ACTUALLY be a harem and is almost always just a mono-romance between the main couple with several other girls as gag / foils / supporting characters. In the classic harem format, the 1st most attractive girl will make a strong move on the main guy in the final arc, which is supposed to fill the audience with tension as the main girl spirals into despair as it seems like he will be swayed by the hottest girl, only for him to reject her in the end and chase after the main girl.
Basically, Japanese media outside of fetish porn is obsessed with moral integrity of the protagonists. They will NEVER make wrong decisions, put looks ahead of feelings, betray, etc. That’s why the main girl can’t be the most attractive, he has to be shown overcoming lust / shallowness. Remember this and you will predict 100% of non-porn harems in JP media.
