Tsucc is not just using the story as a vehicle to get away with the fetish stuff, and he's not just inserting it willy-nilly either. He is writing a story that is well and truly born from the world of his imaginations, and that includes his libido. You're mistaken and do not get how shit works if you think there would or even could *be* a Made In Abyss without the weird stuff. I'm not even going with the misplaced "the lolisho stuff fits with the themes of child exploitation" argument I see so often in defense of it. I don't think Tsucc flopped Riko's titty out for a single panel because pedoshit can somehow be thematically bound up with Bondrewd kidnapping orphans or Belchero not having OSHA compliance. I'm saying the libido is a good thing in and of itself. I think that drawing is probably a masturbatory/tactile process for him, that it's probably why he doesn't particularly like drawing old dudes, and that it translates to things like stone surfaces and tummies and tufts of fur or grass feeling touchable. Apart from the motivation factor of getting Tsucc out of bed, I don't think the art would be the same without his horny. It is legitimately a very good adaptation that is very respectful towards its source material, and the OST is incredible and elevates it significantly.
That said, some things are cut (mostly supplemental stuff about what characters on the surface are up to), there is a change made to a creature encounter on Layer 3, and the intro arc is ordered a little differently from how it's presented in the manga. Also the anime does not draw female nipples so you sometimes have Barbie dolls onscreen for a second or two.

Two good reasons. First, the design makes them a little more endearing and it's a constant reminder these are little kids (12 is little) who are vulnerable. Just as things are worse than they've ever been with no hope in sight, then comes one last devilish ray of hope. Nanachi who seeks to bargain and trade pity for love, and an audience willing to do the same. Who at that moment wasn't completely aligned with Reg? Willing to give up everything and do anything just to continue on.

Anime not set in Japan tend to have more people from other races and Orth is specifically not the equivalent of Japan, rice is an imported good and their culture seems more European.
