The part about VL Ichigo protecting Orihime has to be my pick. She really didn't as it made no changes to the battlefield, Ichigo has been seen getting so many shoulder slices, a blast through his ribs, eyes cut open, etc etc that he just tanks them. What would a slice from the back of Ulquiorra do? He could easily have dodged it, it could be so much easier for him cause there isn't a girl who can't really fight like him against a god instead that would stress him out.

I checked out the scene with Yammy and the translations said he "defeated" or "took down" his opponents, not that he killed them (which is actually accurate with his treatment of his enemies). He does mentions they died later, but only in how Orihime felt compassion for them while Yammy, who is supposedly their ally, derided them as garbage, not that he personally killed them.
