My understanding is that Shinji reinstating the AT Fields caused her to die. Whatever was binding her consciousness to the flesh was severed when Shinji rejected the tree of life or whatever. But Rei/Lilith as a consciousness lived on but as a sort of ghost from the look of it.

There was SEELE's version, where everyone becomes Tang, them thinking it was the next step in human evolution.
There was Gendo's version. Honestly I'm not entirely sure what's different, but Gendo being at the rudder was enough of a deviation for SEELE to send an army to try to stop him.
Then there was Rei's version, where the final decision was left to Shinji, unfortunately at his lowest moment. But that was also when he made his breakthrough, and his acceptance of individuality was enough to leave the door open.
