I went and read the manga and the bl is the main reason Yoshiki continues to empathize with Hikaru and puts up with its weirdness/horrors despite his common sense telling him otherwise, so it's important imo.

No it would have been a very different type of story with a different tone and meaning.
The story it's set around a gay kid in a small town, very likely the ONLY gay kid in town. He doesn't have options, he it's trapped there without financial or emotional support since seeing his family life it's very unlikely his parents would go "oh, good son, take this money and go be gay in the big city". He was born there and it's most likely he's going to die there..
..and then, like a miracle, comes "Hikaru", an Eldritch abomination that looks and acts like his best friends he was most attracted too (but wasn't gay), an in a twisted monster way needs him as much as Yoshiki needs him.
If Yoshiki wasn't gay, and a fucked up monster came dress in the skin of the girl he liked the it wouldn't be a story about desperation, but a guy who decides to go fuck a monster and the moral would be "don't stick your dick in crazy", very different set up. In one it's the story of someone with no options going for the only thing available even if it's a terrible idea, the other is someone who chose to go by a self destructing path because obsession, or nightmare fetishism, or something like that, not for lack of options.
