AFO was in love with Nana Shimura or at least had some strong desire to have her in some way, possibly he decided that she was worthy of being the mother of his heir and was planning to take OFA from her and then make her his afterwards, perhaps as a form of complete victory over fate, life and nature, etc. The evidence for this is that AFO seemed to carry a seriously personal vendetta against Nana and wanted to hurt her as much as possible, even going to so far as to target her entire family line after her death. Why did Nana go through such a traumatic decision to hide her son? AFO is not known to act like a mob boss, he’s obsessed with his chuuni ideals, not settling personal scores, and Nana should have known him well, yet she felt the need to rip her heart out to protect her son from him. That level of personal hatred is out of character for AFO, certainly in his later years. So what gives? Because I think he had a strong personal investment into Nana, and when he learned that she had gotten married and pregnant he went into an apocalyptic rage, who knows what kind of mass destruction he did lashing out. Nana got scared shitless by this for the sake of her baby’s saftey, went into hiding and officially cut all ties with her son afterwards. If she was THAT scared for the child’s safety she wouldn’t have gotten pregnant to begin with. She had no idea that AFO would feel cucked, she clearly didn’t know what kind fo escalation would happen. So she cut ties with her son, and then went to face AFO and stop him at the cost of her life, and even after killing her AFO wasn’t finished with her and made SURE he would ruin her family to the very end of the line and take the successor from it that Nana had denied him in life. “Now who’s the cuck, BITCH” sort of thing.

Nana was clearly a tipping point for AFO. It could be because she was the first and only female user of OFA and AFO got super weird about it like you say. But I have an alternate explanation:

Nana might have been the for OFA user that AFO actually had difficulty fighting.
OFA got stronger over time and with more quirks being added onto it, and the history of the previous OFA users is NOT one of 'worthy rivals' for someone like AFO. Like, User #4 knew from the start that he could never possibly take AFO in a fight and spent his entire life in hiding training up his power purely so that the users that came after him would have a chance. The user before Nana's big accomplish was starting a 'resistance movement' against AFO, meaning that he also wasn't strong enough to fight AFO by himself and clearly his resistance was crushed.

I think that Nana was the first time that the power stacking on OFA finally reached a point that AFO and OFA clashed... and AFO didn't *win*. They fought multiple times, clearly Nana survived their initial encounters. And maybe THAT is what drove AFO to treat her differently and use different tactics on her. This was the first time that 'the hero' to his villian story actually put up a fight and he wasn't able to effortlessly wipe the floor with them. NOW it makes sense that he would go after her family, because for once going after HER was difficult.

As for your other questions, the fanbook cleared some of this up. When Nana received OFA the quirk was already strong enough to be painful for her to use, but it was only with All Might that it became powerful enough to catch AFO off guard. Nana was able to escape him several times, but not seriously hurt him.
I'm of the mind that Nana being a woman did trigger AFO, though more in the mommy issues than the sexual sense, and only subconsciously. The main reason he trash talks her and tracked down her family was to hurt the person who posed real danger to him though, All Might. He also targeted Touya, who had no connection to Nana but did have an indirect one to All Might.

It’s very possible that AFO simply got really weird over Nana being a woman, the only female OFA holder. But I maintain that going after her bloodline down to her grandson to turn him into his successor is more than just working out his weird mother issues. Maybe he actually loved her in some way, since he’s a human being not a mythical creature, and maybe he sublimated that into being weird about Nana specifically. But I keep coming back to how Nana felt like she had no choice but to completely cut ties with her son, a decision which clearly tore her heart out. If she was THAT scared of AFO hurting her children she wouldn’t have had any to begin with and used it as motivation to defeat him before getting pregnant. But she got pregnant anyway, fully knowing AFO was at large and gunning for her as the holder of OFA. Why would she do that? This is just speculation, but I believe that it’s because before that point there was no indication that AFO would hurt her children. It’s not his style. For example, why did the adults put so many UA students on the front line facing AFO in the final war? Because, the students were more-or-less safe from AFO; sure he might scare them but deep down, All-Might, Eraser Head and the other faculty from that era didn’t think AFO would harm kids. And they’re right, because AFO didn’t actually hurt any of the students, even though he could have slaughtered them. All-Mighty and the rest know him not as Satan, but as a chuuni old man who’s dedicated to his larp, someone who wants to turn the world into his vision of a comic book. He’s not normally a hardcore killer, so the UA adults sort of just “trust” that he won’t kill the students. So then WHY was Nana so terrified of her son being targeted? Because I think AFO must have lost his mind at finding out about Nana having children, and lashed out savagely, wary beyond his normal behavior, and Nana got spooked.

Her husband joined En's group together with Gran Torino at some point, possibly years after Kotaro was born, and kept it a secret from her. She only found out about the group and his involvement with it when he died. His dying wish was for her to join in his stead, which is how she happened to be close by when AFO bisected En. Once she became the next OFA successor she immediately gave up her son. This was all described in the fanbook.

Sasuke and Bakugo are nothing alike, they both have shitty personalities but they're basically polar opposites. Sasuke was typically cold and calculating, really only losing his temper when it came it Itachi, which is reasonable considering Itachi murdered everyone he was related to and then used hypnosis magic to make him watch it happen over and over again.
