Mom had them dress the same to prevent Bonaparta from knowing which twin is which. In the end, mom just up and just tossed the girl to Bonaparta instead to be brainwashed in the Red Rose Mansion along the other kids. This fucked up Johan's perception of self and reality however because he couldn't tell if he wasn't chosen because her mom didn't know it was him and not Anna. In the end the mom just abandoned them anyway, and after Bonaparta turned against the brainwashing project and killed the guests there, Anna returned and told Johan of what had happened before they both started their trek across Germany.
Having lost his sense of self he simply imbibed or took on the shit that was taught to Anna for himself and became the monster with no name. But couldn't do it completely since he still cared for Anna and didn't want to hurt her. It's why he later begs her to kill him when she found he's been killing their foster parents.
Basically, crossdressing fucks children up and is more harmful than brainwashing.
