Plain kind of means the same but almost feels like an insult.

I think artists need to make an effort to represent that too, if you just draw a character with the same face as everyone else, plus most boring generic hair style, you are not making an effort showing how beautiful the character is.
pic related is a good example of showing canonically not plain characters, both hikaru and madoka look gorgeous, in spite of simple design. The eyelashes and the beauty mark on hikaru, and the green eyes and cute hair bangs on madoka.
