Not every recurring element is a sign of a fetish. It can be an emotional thing the artist is working through or just something they enjoy normally. You need to be familiar with the person's work before you can tell if the obsession even exists, since sometimes it can be an editorially decided theme the mangaka is only executing as a one-off.
