In her moment of weakness, she genuinely would not have cared if thousands died, just so she could see them again. That was the core conflict of that arc, of Tsunade being forced to decide between her own happiness (which would directly lead to the destruction of Konoha and leading to countless others ending up like her when they lost their loved ones), or letting go of her past pain and accepting her destiny as protector of Konoha.

Yes that was Orochimaru's angle to exploit her past, but Pain's plan doesn't require any personal insight, just forwardness. Gathering the tailed beasts that her grandfather foolishly passed out to their enemies, correcting this by gathering them and maturing humanity, she gets paid to see this through. No more ninja wars, no more fools trying to be hokage and getting slaughtered.
A blackpilled Tsunade might take this offer up, she'd get paid to gamble anyway.
