If it's matter, then there is actually universal measures of value when it comes to mass and energy. That's why alchemy works based off of the energy of tectonic plates (or if you're western, unknowingly, the hellbeast sprawled over said source of kinetic energy).

The thing that's being exchanged for is "Truth". The farther you are away from it, the bigger the price you have to pay. Ed was wiser than Alphonse, and both were much more foolish than Izumi. There's also an element of subjective self-imposed punishment, since what Ed lost isn't objectively greater than what Izumi lost.
