I think at this point about half of all episodes are filler, so progressing through the story is even slower than in the manga.
As for alterations, the anime keeps it pretty straight, there is exactly one plot relevant case that got changed a bit early on and they made sure to never repeat that.
Small character interactions and small changes are sometimes added of course, but nothing major afaik.

Finished Movie 10 in my Conan rewatch lately. My thoughts so far

Great Tier: Movie 1 and 5
Good Tier: Movie 2, 4, 6 and 10
Meh Tier: Movie 8 and 9
Shit Tier: Movie 3 and 7. Movie 7 was especially poor which was disappointing

The anime is very well adapted outside of one very notable case super early on that was altered and they had to change a later case to fix it but that's the only case.

The filler is ridiculous but it is a procedural detective show anyway so if you're not here for that, you shouldn't bother since it's not like the actual story moves that fast. Also some of the anime original cases are as good as the manga. But they start decreasing in quality as you approach more recent years.

There are several examples that people would hardly describe as detectives, so it's not like that part matters.
It's possible he is avoiding Batman cause he is hugely popular in all circles, so he'd hardly need recommendation (Holmes and Poirot are also hugely popular, but you can't have the list without most famous detectives). Or he couldn't get approval to use his likeness. Another possible theory is, since the list is very heavy on japanese characters, it seems to be very reliant on what they currently read/watch (or were told to advertise), and maybe Batman is simply not among it.
