So I started lewd modding sims a while back. I'm assuming you're using reshade? It's likely the depth buffer fucking up the ambient occlusion shader. Open up the Add-Ons tab in the reshade settings and try a different check box. It's usually one of the other ones highlighted in blue. If you're not using reshade then I don't know what's causing that.
