Adding on to this, the insomnia isn't helping, it's gonna make it worse. Work on trying to fix that. Medication or behavioral changes.

I apologize if I'm just regurgitating shit you've already heard but have you tried posture awareness, gentle exercise like swimming or walking, ergonomic chairs, lumbar cushions, heat therapy, even a hobby for distraction?
