First thing I do after importing the image is to open it up in the mask editor.

I usually use a simple setup of epicrealism_v10 and a lora for skin texture. It's not always the best solution but more often than not I can get a decent result within one or two gens.
It also helps to describe the target, skintone, lighting, bodytype and of course what you want to change as accurately as you can.
