It is natural to want or fantasise of things that are extreme, or transgressive - especially if you live in any sort of conservative society, (insert obvious example of the measurably higher prevalence of searches for trans porn in conservative-leaning "red" parts of the states.), that transgression, the taboo is as much part of it as the actual act. and to face that taboo is itself exciting.
and society does mock the idea of a small penis - the boy racer with the massive exhaust goes past, and its "look at him, overcompensating." and so on. And actually embracing and exploring that social taboo, is exciting, in many ways. Chastity's probably the same, the social expectation of virility, to be able to get hard. something which prevents that, is taboo - and the secret of wearing something that denies that function is something that makes it appeal to many.
Personally, I kind of wish there were a temporary equivalent to the opposite of Viagra - a pill that would make you impotent for 4 hours, however much you wanted to be hard. that would be amazing for SPH or kink scenes.
