This needs more than just firing her. Actually, she should not be fired immediately and she should sue, since it could be unjustified if the fault does not lie with her. For example if she followed regular official practice but numbers are fucked for other reasons.

This needs an investigation. The margin of error has been huge there for years. So either the people are incompetent, practices are complete dogshit, or she (and her team) made shit up.
