IMHO, it should be the center of the story. The threat, danger and promise of BE brings a gravity on the characters, pulling them towards their fate. Kind of like a sun in a solar system, or maybe a black hole.

This is opposed to BE as a background element that's forgotten or only brought up once in a while. Then the story needs a different pull, different direction, different fates, etc. manyuu hikenchou is the foremost example of failing to center BE despite its premise, and consequently losing focus and appeal

I've read a few stories set in similar fictional countries and they ultimately boil down to one or the other. Ask yourself these questions
- What is life like in this country? At the top and bottom of the society.
- What does being the empress of it mean?
