Objectively he doesn't. Milo deals with the worst things mathematically possible to happen to him on a regular basis. As long as he gets in close he can kill Tooru fairly easily. Tooru on the other hand really has no idea how to deal with anything that gets through Calamity. Milo solos by making him eat something with Mango (which of course he had in his backpack for such an occasion)

Milo deals with worse things but he has nothing (as far as I know) that can deal with stands so on that side he would be at the complete mercy of WoU.
Moreover he needs to get in close for the colateral of his curse to even affect Tooru while Tooru can eat a burger in the other side of the city and rain calamity on Milo as long as he chases after him (and even if he stops Pursuing WoU could still harass him the same way he did against the Higashitaka family)
Also Calamity has two traits that (in my opinion) would fuck over Milo
The first one being that it bends logic, there's no way for Milo to prevent that a single leaf will cut one of his fingers, or that the rain will suddenly pierce his skin.
Second calamity can work in more ways than just direct harm (Josuke got arrested because of it and Rai got his food full of worms) it is entirely possible that Milo ends up losing access to his backpack or gadgets mid fight thanks to some unfortunate event, while Tooru's ability is much harder to shut down.
The one thing I think could flip the MU is if Milo has some Phineas and Ferb Scaling or something I'm not aware of

I already explained why I think Tooru is likely to win against Milo.
But for Making the logic is a bit different.
The main thing people claim gives the W to Tooru over Makima is the fact that A calamity hit wouldn't trigger her contact (and that's True) but that ignores the fact that Makima's devil biology would allow her to come back to life after ingesting blood, or just use the Snake devil contract in Sawatari's corpse to resurrect herself (these also can heal her if the damage is not lethal) so it's far from a one shot
Plus Makima has a special position to exploit one of WoU's weaknesses: The flow of calamity.
Calamity hits in a specific order, it can't just murder an entire crowd of people at the same time. So if Makima is preparing a ritual against Tooru at 500 kilometers away (or just shooting him with the Gun devil's bang) while all the hybrids are rushing towards him to rip him apart , who gets hit first? Because if Makima gets hit it's likely the faster Hybrids like Katanaman, Quanxi, or Reze instakill Tooru, but if the hybrids get hit first Makima also can one shot him.
Remember that Calamity hurts those pursuing, but it doesn't protect Tooru in any other way. If someone dies because of calamity but they manage to shoot a bullet WoU's calamity won't do anything to stop it.

IMO this is what makes Milo's win likely in close quarters. Milo's negative probability ions would likely cause a good chunk of the damage to come back to Tooru, and unlike Milo, Tooru has NO preparations against calamity.

I was thinking about that. Logically the battle would start with the two of them at a fairly close distance
Something like "Milo accidentally finds Tooru's Rokakaka stash/Trade and now The Rock human wants him dead" could work
The thing is if the fight starts at close distance Tooru's natural course of action would be to run away (that's not even OOC he ran away the second Josuke managed to damage him for the first time) and let calamity do it's thing.
Of course Tooru has no running speed feats and Milo is fairly athletic himself so it's likely for him to catch him up or at least keep the perfect distance for his own curse to affect him
But Tooru still has options, mainly his rock insects.
Dododo de Dadada can chase Milo on its own and if he Lands a hit his joints will be filled with asbestos whenever they extend (making it difficult to run)
Obladi Oblada sticks to it's target and sucks their blood, multiplying exponentially fast weighting them down
Both of these are deployed by WoU so Tooru can go for a surprise attack
And while Dododo de Dadada would be relatively easy to handle for Milo (only needs to squash it) Obladi Oblada is harder to get rid of (if it lands) because it grows all over your body even if you remove the original (Josuke took that off him by bathing on a Sauna or something with hot water so it's not entirely impossible for Milo I think)
These two + Calamity + Milo's own misfortune would weight him down while all Tooru needs to do is run away and avoid a crippling accident.

IMO Running Away is kind of against the point of a death battle. I think Tooru running but Milo chasing him is a good set up, and it becomes Tooru's calamity + insects vs Milo's calamity + interference with Tooru's calamity. Obviously this is incredibly up in the air, but I think the sheer fact that Milo has some preparation, even just mental prep, while Tooru doesn't gives him a HUGE advantage. If anything, the fact that WoU has more potent, yet smaller interactions is worse because it makes them more likely to get caught up in a building demolition or gas main explosion that Milo would be way more likely to cause.
