Bill was a high school football legend and charismatic army recruit by the time he was 18. But he missed 'Nam, got assigned to a subpar support position, and got singled out for an abandoned placebo program.
It's implied that the Dauterives were abusive, inbred genetic dead-ends, so his health taking a nose-dive and going bald probably happened pretty early.
Lenore probably met a slightly less than peak Bill, only to marry him when he was on a full downslide physically, which probably built up a lot of resentment on her end. She expected to be married to a strong, handsome armyman, but got Bill. Since he was already use to verbal/physical abuse from his family, he never really fought back when Lenore started getting abusive. It all just wore down on him until he completely fell apart.
Lenore basically just hit a bomb with a lit fuse with a hammer and then ran before the explosion could ruin her life, too.
