it took me a second to figure out but that's an anthropomorphized version of Plantera, a boss from Terraria. The key she's holding is the Temple Key that she drops when you kill her. Those aren't ears, they're seedlings that she shoots at you during the fight (here, they're being used as hair buns, which is pretty cute). Finally, she's playing guitar because one of her item drops is The Axe, a tool with the highest axe power modifier in the game that looks like a cherry red Flying V guitar (it makes cool guitar sounds when you swing it).
