As always, it is done to the inherent issues of long term characters and their powers and how writers will one-up them. As always I refer to that comic mocking this.
>Character can understand any language instantly
>Starts off as minor one off character used as comic relief for main people
>Character gets popular, so writer ups his powers by stating he can 'read body language' since it's a 'language' this makes him a god tier fighter
>He becomes a side character
>Popularity continues, he can now 'hear' areas like cities or jungles or whatever and immediately find important thing because he 'understands the language' of the area
>He becomes a character in his own right, gets his own runs
>Now writers start putting him against galactic level threats so need to up his powers, so the guy can talk to atoms and molecules and tell them to form to his will, he now has reality manipulation powers and can do whatever the fuck he wants as he is essentially a God
>Character is now a main character in his own right
And after that, the writers either forget he could make his own Infinity Gauntlet or handwave 'he got hit on the head and has amnesia' or some shit to make sure he can't just make the atoms inside Galacticus explode or something.

Also there isn't a lot of 'fun' in reality manipulation. If you can do everything, who cares. In the anime Fire Force, everybody with powers has a power that is flame based. They have power over flame/heat etc. This means with some heavy handwaving you can do loads of fun things that are really really really simply correct (if you smash it down really basic) such as 'I can control heat so well I can change the gravity of an object and therefore make you fall into the sky or bend light so you can't see it'. Or 'I run heat through my gold gauntlet, making a non-magnetic metal magnetic and can control all magnetic metals like Magneto'. Or 'I can use my flames that come out of my feet to go really fast, so fast that I can go back in time'. Etc

Telepaths. They are too useless or a win button with little in between. Writers really struggle going half way with them. If they can get in your head at all there is no reason they can't shut you down but if they can't they are useless as tits on a bull. Which tends to lean to the second problem they get new powers that don't make sense. Like Psychic energy bolts, weapons, force fields ect. Psychic never projects physically or behaves like energy until a character needs an offensive alternative and then it does. I don't think I have ever seen an energy manipulator do anything with psychic energy without being psychic themselves. I think X-men corned the market on psychic ghost. Where somehow when they die psychics can live as psychic entity without the need of their brain.

The question is, who is in charge: the character, or the power?
Superpowers have a way of exerting a will of their own, and the greater the power, the greater the will:
>On one end, you have a character like Batman, who is basically powerless and does what he does through his own sheer force of will.
>On the other end, you have a character like Galactus, who is less a person and more a force of nature. His power IS who he is.

Reality warpers fall on the Galactus end of the spectrum. It's hard to write them as characters when they're defining the setting they're in--in effect, they are the setting.
