They're not transphobic jokes because for that to be the case, the joke would have to be that the butch woman actually IS a man. But that isn't the case. Does Bungus not really get that Smiling Friends can't have any important female characters in the group dynamic for the same reason that Venture Bros never did? If a story focuses on a very small single-sex cast, intentionally adding any opposite sex characters to the dynamic would break the story because specific male characters in the existing group dynamic already function as women (not literally, but in terms of psyche and the roles they play in a team).

There's no point in adding a girl to the Smiling Friends because Pim is pretty much already "the girl". And maybe Alan if you squint.

Having female clients isn't what Bungus is suggesting, Bungus is suggesting that there should be more female characters in the main cast. Although yes it would be nice to see more female clients, it looks like Zach and Michael are stuck in the same trap as the Boondocks creators where they're good at writing believable characters but don't feature women all too often because otherwise they'd be crucified for depicting women as anything but perfect harmless blameless angels.

I think a lot of people straight up aren't aware that the "hulking nurse who looks like a man" bit is a joke that was pulled straight from the post-WW2 landscape of hospitals where a lot of nurses genuinely were fucking straight up YOLKED because being a nurse back then was tougher and more violent than it is today. And even these days, a lot of nurses are quite physically fit or strongfat.

That's why the juxtaposition is with the sexy nurse visual: the sexy nurse was something more or less invented by wartime propaganda, porn, and mass media.
