IMO
Transvestite and hermaphrodites are "real-world" terms that mean real things
Futa is a made up "fantasy" term that has no real world equivalent and more or less means what group consciousness want it to mean.
more specified variation of "futa" would be said "dick girl" which would be basically fantasy version of transvestite, female body with male genitalia. but when you are talking about hentai, transvestite =/= dick girl. transvestite would specially be a male who has had body modification to appear female when dick girl would be someone who naturally grew into a female body but has male genitalia.
Good example of this would be works of amanoja9 who exclusively draws transvestites and not futas
Full-package futanari would contextually be pretty much a same thing as as a dick girl but also has female genitalia in addition to male. real-world version of this would be the hermaphrodites but these basically do not actually exists and when they do, they certainly dont just grow into females with two working sets of genitalia so hermaphrodites are functionally different thing from full-package futa.
Classic futanari would be a normal female who for some unrealistic magical of sci-fi reason grows a penis this of course is complete fantasy with no actual real-world equivalent
In short how stupid as it might seem, I would say what defines what is a futa and what is a transvestite or a hermaphrodite is completely dependent on a context.
TS = male who mody modded himself into having a female body
Dick girl = female who happens to have male genitalia
Both are functionally same thing but contextually different.
