Scrape, wire brush, sand, feather edges into old paint, Ospho or some other phosphoric acid rust converter, mask off and paint.

>>2930022 (OP) It rusted cause it leaked. You need to fix the leak. I would probably wire brush it, use phosphoric acid to convert the rust, and TIG weld on a threaded piece with real low heat to avoid thread distortion. Then seal it proper so it doesn't leak again.

Drain the radiator, GENTLY remove cap, get now O-ring and silicone grease. Reinstall cap with new O-ring and grease wire brush off rust and repaint.
