Scrape, wire brush, sand, feather edges into old paint, Ospho or some other phosphoric acid rust converter, mask off and paint.

It rusted cause it leaked. You need to fix the leak.
I would probably wire brush it, use phosphoric acid to convert the rust, and TIG weld on a threaded piece with real low heat to avoid thread distortion.
Then seal it proper so it doesn't leak again.

it's rusting because it has leaked.
that cap has an O-ring.
the O-ring has failed.

Drain the radiator, GENTLY remove cap, get now O-ring and silicone grease.
Reinstall cap with new O-ring and grease
wire brush off rust and repaint.
