Reminder that if you can clearly see down to your scalp when your hair is wet, your hair is thinning, lots of small new hairs sprouting at the hairline or crown is a sign of hair recession.
Holy trinity:

Finasteride/Dutasteride are 5-alpha reductase inhibitors that prevent free T from being converted to DHT, a byproduct of testosterone that serves no purpose after puberty but to choke out your hair follicles and make them thin out and die in a few short years, a condition known as androgenic alopecia.
5ar inhibitors prevent your hair from getting any worse, mild regrowth is expected, rapid onset diffuse thinners see a significant recovery on fin alone. Can be taken orally or topically.
5ARIs are not dosage dependent, taking as little as as 0.2mg daily fin can reduce serum DHT by 68%, while 5mg reduces by 72%.
0.5mg daily dut reduces serum DHT by 95%, but is considered a second line treatment, and can be more expensive and harder to get prescribed. Start with fin.
Fin has been used to treat benign prostatic hyperplasia (BPH) for 30+ years, it's very well studied and is tolerated well in almost all men, however, there's a 1-2% risk of erectile issues in controlled studies.
These can be avoided by reducing dosage, spacing out dosage, or stopping the drug. Fin takes days to fully bio-accumulate, the half life of fin is about 8 hours, adverse effects occuring within minutes of starting fin or persisting long after stopping fin are not real and have no basis in reality. Please note that dut has a much higher half-life of 5 weeks, if you're prone to fin sides, dut may cause them for far longer.

Minoxidil (liquid and tabs) is a vasodialator that opens potassium channels and widens blood vessels bringing nutrients to your hair follicles, stimulating them into thickening, accelerating growth and elongating the growth phase. Min is taken with fin to restore your hair to it's original hairline, thickness and vigor when hair loss is caught early enough, (pre-NW2). Can be taken orally or topically.
Topical min does not work in 1/3 of men because they lack the required enzymes in their scalp, all men have this enzyme in their liver so the oral route is a good fallback.
The FDA formulation for 5% liquid minoxidil is food safe (vape juice, ethanol and water) and can be safely taken orally if oral min tabs are expensive or rarely prescribed where you live. As a general rule, ~4 drops (0.1ml) = 5mg minoxidil.
There's a 1-2% risk of fluid retention with higher oral dosages, start with the lowest dosage possible and work your way up when you're sure it's well tolerated.
Ketoconazole is an anti-androgen and antifungal that inhibits DHT on the scalp and used to treat fungal infections of the scalp that may be making your hair loss worse such as seborrheic dermatitis, a condition where you exhibit severe greasy, flaky large dandruff flakes with dried yellow crystals (blood plasma) embedded in the scalp. Some people may only have traction alopecia (hair loss from scratching or friction), but it will not cure androgenic alopecia on it's own. Can only be taken topically.
Do not use spirolactone. Spirolactone is a diuretic which, in large doses, inhibits testosterone and, by extension, DHT.
It is regularly prescribed as an anti-androgen for male to female HRT, and thus, comes with an astronomically higher risk of sexual side effects in men including ED, gynecomastia, and can cause potassium toxicity.

Ideally you should just be aware and not compound the issue by doing that shit like an addict. In the grand scheme of things proper hygiene should be sufficient enough to not have to worry. Just shower after you jack off, workout, heavily sweat. It's a physical substance on your scalp which can be washed off with water.
I got a hair transplant
I can't believe I put this off or was afraid of it. I spend more money fixing random shit around my house each year than I did on this and it was easy. I'm way early in the process and will go through some ugly duckling stages but holy shit I can't believe how easy and honestly relatively painless this was.
The rest of my fucking life I don't have to worry about my stupid fucking hairline anymore.
I am on meds btw. I have been on fin for like 8 years prior to this so no stranger to it. That was the first smartest decision I made, stopping recession in its tracks which made it not too many grafts at all. Now this is the second best, restoring to a youthful but realistic hairline.
THANK GOD I never listened to any faggots trying to scare me out of using fin. THANK GOD I pulled the trigger and just fucking went to this.

Just get fin online, there's lots of online services that can give you prescription without questions, and ship you a supply every month.
Mine just asked for pics of my hairline, then said ye you're balding, here's a prescription. My hairline wasn't too bad to begin with either desu.
The 1mg fin is usually expensive, so more commonly guys will get the 5mg fin that's meant for enlarged prostate, and cut a pill into 4 pieces and have a quarter everyday. Most online services know this and will have an option for the 5mg and will even give you a pill cutter. Comes down to ~USD$12 every month.

Can retinoids somehow affect hairloss? I know it sounds extreme but I'm lately thinning extremely hard even though I've had good gains with min&fin, I'm talking big hairballs whenever I comb and/or wash my hair daily. My stress levels feel the same and I didn't significantly change my diet or sleep pattern so getting back on retinoids is the only change that comes to mind. Is it possible the pharmacist forgot to add costisol to the min mix per my derm's prescription and it's now biting me in the ass?

Sebum is important for hair loss because it involves DHT.
Enhanced sebaceous gland activity is attributed to the potent androgen 5Ξ±-DHT5 as sebaceous gland cells possess all necessary enzymes for conversion of testosterone to 5Ξ±-DHT

so this is the information online:
- sebum contains dht
- sebum is produced by sebaceous glands
- sebaceous glands are attached to hair follicles
- these glands are present all over the body, with most of them on the face and scalp
i think the math is adding up
while there may not be actual studies on this, it doesn't take a genius to figure out hair loss may be directly influenced by masturbation and sweating
although, i am not sure if the sebaceous glands contain less dht if your blood stream dht is already low, perhaps due to strong anti-androgen activity going on such as being on higher doses of dutasteride
but it seems like dht is produced in the scalp, so even if your body doesn't contain high levels of dht, you could still get fucked over by 5ar producing more dht on the scalp itself

7 months in, i still lose hairs (about 200-300 or maybe even more) on my wash day once per week
it's fucking insane
i never used to get so many hairs in my drain before, now it's a weekly occurence and my scalp is so noticeable unless i part my hair to the sides
no regrowth either, i don't know how long i have to wait for my scalp to fill up again
it's honestly discouraging that you can go so high on your dosage and your shitty genetics will still spit on your face

Seriously considering getting a beard to hair transplant along with scalp hair transplant to help with density. I entirely shave my beard hair below my chin and jaw anyway and it could really be used to thicken up the crown area of my head if spaced out intelligently alongside actual scalp hair.
Could get 1-1.5k additional grafts for density. That's kind of a big deal. Just gotta find an HT surgeon who has experience with beard to scalp transplants.

After that, wash with antibacterial shampoo (not antifungal) and then apply the minoxidil. Iβd even recommend an oxygen bed at a spa to activate the process.

Less intensive than taking a pill? Not even close. You would need to adopt massive lifestyle and diet changes in the hopes of reducing DHT production. It would likely be in vain anyway. Anyone from germany and/or have experience with ru58841?
Would ideally like to get finasteride/dutasteride, but am scared shitless of side effects, especially because I've seen so many people be left with them *permanently*.
topical ru58841 seems like a good alternative, especially because you can't get fin without a perscription here in germany and the only doctors that give it out are online and I don't want to put my insurance info out there. Can someone give me pointers or advice?

I've been on fin for 8 months now and I don't know if it's because I have long hair and I wear headphones all day but my crown is in pain a lot. It feels tense and scratching it feels uncomfortable as opposed to the sides and back. Could I be experiencing traction alopecia or some other kind of balding? I am also underweight. Maybe buzzing it will relieve some of the pain.

How often do you shampoo? Depending on your hair type, you might not need to shampoo every day. I only use shampoo twice a week and it's the sweet spot where my hair are neither dry nor greasy (of course I'll shampoo more as necessary if my hair are dirty that week).

Your dermatologist doesn't know what she's talking about. Minox compensates for hairloss by making hair grow faster. Fin mitigates hairloss by making hair fall less. If your hair growth can't keep up, then you clearly need to address the other side of the equation. Consider a second opinion.

There is mountains of evidence in the skincare world directly linking acne and oily skin to DHT through sebaceous glands. There is no evidence in relation to balding because it hasn't been studied but the same rules would most likely apply from the skincare world. At least she gave you oral min. I've been using fin and min (topical min) for years, had to stop for around half a year to have a kid (yes, fin fucks up your sperm and it's even written on the fucking leaflet that comes with it). Went back on it afterwards but started drinking min instead of topical use, the difference was night and day. I regrew at least an inch on my temples which I thought was long gone and finally could rid myself of long cope hair.
At least where I live, they make you jump through a million different loops to get oral min, so making the jump to drinking it was the only way, it was well well worth it. Truly a secret they keep hidden since it works so well and it's cheap as fuck.

there is no way in hell an enzyme inhibitor has permanent side effects. You stop the medication and DHT comes back. Now, if you get traumatized because you couldn't get hard once or you had "mental fog", then my money is on anxiety that is well beyond finasteride's reach. There is more mentally crippled people out there than most would think.

Benzos work miracles for people with crippling anxiety, but it's very much stablished that they are

they have "better reviews" because few things work better to instantly erase anxiety than benzos/alcohol, not because they are not toxic shit.

My temples haven't moved and are still full of hair, but the thin top is clearly visible when it's dry. Should I get a dermapen to get some blood flowing on my top? Or am I paying too much attention?
I've been through long periods of stress/depression and everyone on my mother's side is terribly bald/big forehead, even the women, except her who somehow got chadlike genetics from some distant ancestor. I'm not sure if it passed on to me or if I'm cursed like my other relatives.

It looks like you have clearly receded significantly since the first pic. The right temple is receding further than the left as well which is common. You need to get on finasteride first and foremost then worry about regrowth once you have the underlying problem under control. Otherwise you need to accept that you will be bald eventually and no other treatment besides a 5ar inhibitor will be able to prevent that long term.

I have an older photo where there's already a clear Z shape there, barely hidden by my hair being pulled back instead of being mid-long-ish. And my temple hair is very thick and close together, there's no "see-through" area like in the front, which is why I never worried about them.

No I don't think so, you are balding and somewhere between a norwood 2 and 2.5 currently based on the last photo. You don't even seem to have a visible temporal point on the right side (my right).
High testosterone is not at all the cause of male pattern baldness, it's a hormone called DHT which is present even in women.
It's not radical, it's the front line treatment and the majority of people who decide they don't want to go bald take it. This is millions of people, it's not some experimental treatment.

PP405 is a prodrug of the patented chemical JXL-069, specifically the formulation labeled PP20 in the 2024 patent US20240327400A1.
There's a synthesis pathway included therein, or you could get a lab in china to synth you like 15 mg as a year supply. Once you get the shit, you need proper compounding equipment to create a 0.05% mixture in PLO Transdermal Gel. Apply topically once a day and you'll have your hair back to a norwood 0 in about a month with no side effects.

don't want to take drugs for it since I'm not really losing anymore hair and I read that the drugs won't bring the hair around my temples back anyways.
still wish I sought treatment immediately hwen it was first happening.

anyone here has an anecdote on microneedling working without topical min? I've been on 0.5mg dut and oral min (initially 2.5, now 5mg) for about 10 months, got good results but I still have a big amount of miniaturized hairs (not vellus), they have color and vary in thickness.

Think of it this way. Balding has fucked over men for tens of thousands of years. Genuinely, TENS OF THOUSANDS. Since we were cavemen. Julius Caesar was insecure about his balding.
If there was legitimately a reliable cure for baldness that existed before modern transplants or drugs, it would have been everywhere and we would have grown up taking it as easily as we were taught to brush our teeth. It's like contraceptives, something a huge amount of the population desperately wants. The fact that we never found it is proof enough to me that it never existed.

We're talking about two completely different applications. If the balding was caused by an injury impacting the surface tissue then maybe you could compare the two? Forcing your body to heal your scalp isn't going to magically repair your miniaturized follicles, if this was the case balding would have been cured a long time ago with a simple head trauma once over. The only thing that's actually going to happen is an increase in blood flow in response to the injury, like a watered down minoxidil. In fact standing on your head for an hour would be better if that's what youre after.

I tried to take some photos, first one is of the back of my head - you can see that the hair at the back of my head is of uniform and higher thickness, while there are a lot of hairs somewhat shorter than my actual hair on the top.
I presume these hairs have started growing at the same rate as healthy hair/deminiaturized when I started doing fin due to their length, which also makes it hard to take a photo clearly showing a distinction between them and healthy hairs, and I'm pretty sure I can feel a mild improvement in density by running my fingers through my hair and it seems like there's less scalp showing but I do have slightly longer hair now than before I started.

thank goodness for telehealth and otc rogaine, I don't think I could deal with constant doctors visits and blood tests just to get 30 days worth of pills at a time, what a fucking nightmare

yes, start now
So transplants, even if you get one you still need to take Fin/Duta right?
I could be saved without a transplant with Fin/Duta right now but I fully believe PFS sufferers and don't wanna risk my health.

You will try anything when your balding gets worse and at that point it won't be nearly as effective as it would have been. Many such cases.

I bet you won't. 1 more norwood and you will rush to every med available to try and save it. Nobody has a non functional dick you are either reading fantasy or your entire story is a lie so that you push this tired narrative about how righteous you are.

Get bloodwork done at least before ruling out one of the most effective and proven medications. It's really sad to see young men in their prime norwooding because of some schizos on reddit threads taken at face value.
I bit the bullet and it ended up being the best decision I made

No it's not.
You're going to ignore a whole class of medications because some mentally ill retard told you their skull shrunk and their skin stopped producing sebum after taking just one fin pill.

SSRIs and Benzos can also cause permanent issues in a small amount of people.
The going theory for all these is a genetic defect of some kind, honestly hope they can isolate and screen for it within a few years. Knowing if you're one of the rare few predisposed to PFS/BIND/Whatever you wanna call SSRI damage would be innovative as fuck and save lives.

the only notable source of sides is from extremely low-T men getting gyno after the increased testosterone aromatizes to estrogen, I don't think TRT would save them either

PFS isn't a diagnosis nor has the medical community even identified it as theoretical lol. Just lump all of these conditions into severe psychological delusions and don't prescribe crazy people anything besides anti-psychotics.
