Reminder that if you can clearly see down to your scalp when your hair is wet, your hair is thinning, lots of small new hairs sprouting at the hairline or crown is a sign of hair recession.

Finasteride/Dutasteride are 5-alpha reductase inhibitors that prevent free T from being converted to DHT, a byproduct of testosterone that serves no purpose after puberty but to choke out your hair follicles and make them thin out and die in a few short years, a condition known as androgenic alopecia.
5ar inhibitors prevent your hair from getting any worse, mild regrowth is expected, rapid onset diffuse thinners see a significant recovery on fin alone. Can be taken orally or topically.
5ARIs are not dosage dependent, taking as little as as 0.2mg daily fin can reduce serum DHT by 68%, while 5mg reduces by 72%.
0.5mg daily dut reduces serum DHT by 95%, but is considered a second line treatment, and can be more expensive and harder to get prescribed. Start with fin.

Fin has been used to treat benign prostatic hyperplasia (BPH) for 30+ years, it's very well studied and is tolerated well in almost all men, however, there's a 1-2% risk of erectile issues in controlled studies.
These can be avoided by reducing dosage, spacing out dosage, or stopping the drug. Fin takes days to fully bio-accumulate, the half life of fin is about 8 hours, adverse effects occuring within minutes of starting fin or persisting long after stopping fin are not real and have no basis in reality. Please note that dut has a much higher half-life of 5 weeks, if you're prone to fin sides, dut may cause them for far longer.

Minoxidil (liquid and tabs) is a vasodialator that opens potassium channels and widens blood vessels bringing nutrients to your hair follicles, stimulating them into thickening, accelerating growth and elongating the growth phase. Min is taken with fin to restore your hair to it's original hairline, thickness and vigor when hair loss is caught early enough, (pre-NW2). Can be taken orally or topically.

Topical min does not work in 1/3 of men because they lack the required enzymes in their scalp, all men have this enzyme in their liver so the oral route is a good fallback.
The FDA formulation for 5% liquid minoxidil is food safe (vape juice, ethanol and water) and can be safely taken orally if oral min tabs are expensive or rarely prescribed where you live. As a general rule, ~4 drops (0.1ml) = 5mg minoxidil.
There's a 1-2% risk of fluid retention with higher oral dosages, start with the lowest dosage possible and work your way up when you're sure it's well tolerated.

Ketoconazole is an anti-androgen and antifungal that inhibits DHT on the scalp and used to treat fungal infections of the scalp that may be making your hair loss worse such as seborrheic dermatitis, a condition where you exhibit severe greasy, flaky large dandruff flakes with dried yellow crystals (blood plasma) embedded in the scalp. Some people may only have traction alopecia (hair loss from scratching or friction), but it will not cure androgenic alopecia on it's own. Can only be taken topically.

Do not use spirolactone. Spirolactone is a diuretic which, in large doses, inhibits testosterone and, by extension, DHT.
It is regularly prescribed as an anti-androgen for male to female HRT, and thus, comes with an astronomically higher risk of sexual side effects in men including ED, gynecomastia, and can cause potassium toxicity.

Am I cooked niggas? I spoke to my doctor yesterday. I was under the impression that I was on month 4. I told him I was still shedding, like if I pull in a random part of my hair I'll get 1-2 strands, long with bulb or sometimes miniaturized, then I'll try some more and that area is finished. Try another area, and same thing. Repeat all over the hair and I can end up with 40-50 strands as I do this. Later in the day, I can do it again (maybe not as effective but still, one time I ended up with 80 strands doing this right after a shower)
Doctor said that Finasteride could be ineffective for some people and asked if I wanted to stop

I know what the problem is. Without hair, there's just no "stop" at the top. Like, you don't know where the person ends. That's why you look unfinished as a person and like something's lacking at the top.
In jason's case, his remaining hair actually perfectly frames his forehead, so his forehead IS the end. That's why it looks so good on him. If you compare it to vin diesel, for example, you can see immediately how it just looks like an unfinished person. Like something is missing, even though overall he looks great as well.

I think that's why beard goes so good with balding. The balding becomes the mark of the end, it doesn't become a continuation of the bald chin. Bottom part is hairy and the top is bald and finished.

In Jason's case, he has a stub that works perfectly with the stub on his head. This frames his forehead. He looks like a finished person.
So the idea is that people like pic related could earn a lot by trimming their beard down and leaving some stub on their head, and styling the sub so it frames the forehead. They would probably look a lot more handsome that way.
Like this, it just looks inconsistent and unfinished.

no, the anti-androgenic effect of spiro is just a side effect, it's used off label, and for amounts strong enough to force feminize you, you'll be pissing all day and night, there's better antiandrogens available, though these are strong enough that they WILL force fem you and give you osteoporosis in the absence of estrogen.

i have oily straight hair, increasing hair density with minoxidil really helped, but so does:

I feel like the conditioner delays how long my hair will stay semi dry before it gets matted down and flat and oily.

What helps me is putting min onto my scalp some time after taking a shower (and using some sort of conditioner or hair mask when showering). I cover the whole top of my head with min and let it dry, works well enough to keep my hair dry for at least a whole day, sometimes even two days.
That being said, there's no real method to keep hair like ours non-oily.

For how long? If you are not already: Dont stray from the prescribed dosage, 1mg daily, no skipping days. Look into oral minoxidil if it has been 6+ months without tangible results, you may be a non responder to the topical form.

You have made progress but the lighting makes it impossible to tell. If you can get a picture with the same harsh lighting that would help a lot.

Time and consistency is key. This is what you should do: Stick to the regimen religiously for 6 months. The hiatus from fin will make gauging the effectiveness of the past 6 months of min skewed. After those 6 months if your hair is the same as it is now definitely consider swapping to oral minoxidil which will synergize with fin yielding better regrowth. In the meantime trust the process, you will probably doubt it at times especially if a shed occurs but you need to stick to it with that consistency.

How long does it typically take for dutasteride to start slowing down my hair loss? I've been taking it 3 times a week for about a month now but I haven't really noticed any major differences so far despite supposedly being more effective than finasteride at blocking scalp DHT levels.

I started noticing at around 4 - 6 months in.
Also I was doing 0.5mg daily, and if I missed a day or two, my next dose would be 1 - 1.5mg. I have very aggressive diffuse thinning though, so if you have moderate hair loss, maybe you won't need to watch your intake that much.

No it's not common. The hair loss pattern you are experiencing is not typical for male pattern baldness though not impossible, your age makes it double unlikely.

You should try an anti-fungal shampoo like with ketoconazole and change the current ones you are using. It seems to me like you may have a fungal infection or your current shampoo is irritating your scalp.

nope, we have the same hair stylist (he should just buzz it at this point, my mother and I are persuading him to at least try min) and he told me that he's never seen a balding pattern like that and reckons it's stress shedding

Significantly lowering a hairline with meds is very unlikely because of how long the process of receding is on average. The follicles are usually beyond repair by the time you have gotten into advanced norwood stages. Thinning crown comes in way later for most norwooders who are not diffuse thinning from day 1 so it's usually responding far better. The most you can hope for with oral min is going down half to a full norwood, beyond that is transplant territory.

Topical fin sounds great on paper. I'd love to give it a try some time but just having a pill and not doing anything messy with your hair is just too convenient.

You should run a trial of finasteride at least, just stop taking it if you run into problems, which in all honesty you won't. If DHT blocking is ruled out completely then you will need to focus on increasing blood flow to your scalp. Scalp massages, micro needling in conjunction with topical min. I've heard talk about caffeine applied topically increasing blood flow too. You should include oral min as well for optimal coverage, even low dose will help. You can slow it down for a while like this but you will lose ground on average after the initial boost.

It gradually decreased these past few years from hypersexual levels to below average(but still functional).

Depends on the density of your donor supply. You don't have enough follicles back there to transplant your entire scalp so you will need a strategic plan from a good surgeon to map out that limited supply. Imo the safest bet is to assume the worst, which in your case might be early signs of androgenic alopecia. Monitor your thinning spot every month or so, it looks like your crown might be starting to go. Telogen effluvium is not that common and traction alopecia from a gaming headset is a meme unless the hairs are getting constantly tangled on the headset and pulled from their roots repeatedly, which is highly unlikely. Always assume the worst and be proactive when it comes to your hair because it's way harder to get it back than to stop it from going/slowing down norwood.

Don’t use fancy shampoos. The best ones are the most basic dollar store shit like Head and Shoulders or even Johnson’s baby shampoo. The reason is because shampoo should be very simple meaning it’s an antibacterial. No added oils, no added chemicals, no bullshit like avocados and coconut. The primary use for fin is to halt the progress of balding and maintain what you have with regrowth expected for a little over half of users though the amount of regrowth varies. Regrowth takes a while with fin alone, 3 months is way too generous to be expecting anything of note. Think of it as balding in reverse, your hair didn't get to where it was pre fin over night. Min is a growth stimulant which can help speed up results, optional but highly recommended. Everything else is a meme or will result in marginal gains at the very best. Dutasteride is just a stronger version of fin for people who want more out of it. Nothing wrong with being on only fin, your balding is halted and in time you should get some regrowth.

Not worth it without using topicals. I've theorized that given enough time DHT levels should naturally decrease alongside test. Since fin gains are primarily the result of reducing DHT and not a stimulant then its possible that stopping at an older age should in theory result in retaining some of those gains or at least allow them to remain for an extended time. Basically you restart balding but at a much slower rate compared to 20s and 30s and maybe DHT is naturally low enough now for it to be minor.

I think I'm beginning to suffer side effects from Finasteride. I can still get hard but I lose it very fast. I used to nut several times a day (5–8) but now I barely have the interest to do 1. So dermatologist said most of my balding is telogen effluvium from rapid weight loss, I do look to be receding a little but nothing is shedding in the front, just patches around my ears which they say is the TE.

They recommend I stay at maintenance for awhile, so I guess I'm gonna lean bulk and lift harder even though I wanna be leaner.
