He didn't train at all for the first movie, or very little. He got the role right before shooting started so had almost no prep time. That's why he looks like relative shit in the first one. Much more prep time for the later movies (and gear, of course).

Hugh especially took it to an extreme with his dehydration/fasting and it followed Brad Pitts' Fight Club look.
Which is just extreme workouts 2-3 days before shooting and no food.

I think brad pitt specifically did his workouts right before shooting, since they lived in that shitty house and were not bathing, etc he didn't have to have the makeup look so they just degreased him a little.

Its just Hollywood trends. Muscles in movies were more popular in the 80's and now than they were in the 90's-2000's. In the 90's-2000's, muscular guys were seen as cheesy, because steroid use was very stigmatized compared to now.

It started with 300. Chick flicks had nothing to do with it. No men watched this shit and if they did they would hate this guy. It wouldn't motivate people to workout or want to look buff.
300 came earlier and it made men completely obsessed with being muscular. It may to this day be the most insane abs obsessed movie. People were painting abs on their gut and wearing fake abs under their clothes after 300 came out.
Before 300 large muscles would be seen as something freaky that only men like Arnold can have. That proto-capeshit movie put muscles in the mainstream and made people think every person should look like that.
