The most significant determiner of one’s weight for the overwhelming majority of the population is hormones that determine hunger and satiety (like ghrelin and leptin). Skinny people are skinny primarily because they don’t eat much, which is because they don’t feel as hungry as people fatter than them, which is also why GLP1 drugs work so well. A distant second reason is NEAT, or involuntary energy expenditure like fidgeting, that skinny people have more of, but it is still far less important than hunger and satiety for most people.
