I mean the ISP can see what IP addresses you're connecting to. The IP doesn't mean as much in the increasing number of websites where you actually connect to a generic Cloudflare IP or Azure or whatever, but by choosing to use Cloudflare DNS, you're just letting big tech see what you do on the remaining part of the internet they don't already control.

you need ECH (Encrypted Client Hello) for TCP too. Otherwise, they would have to guess from the IP address, which may not be super hard, but not trivial, and would only work with big sites (known IP blocks), or sites with matching reverse DNS records.
but if you're really bothered, you're using a VPN already, and doing that dance with the VPN* provider just in case, instead of your own ISP.
