Actually, as an instance admin you can block other instances (this process is called defederation).
By default every instance can communicate with every other one, defederation means that one instance cannot communicate with another.
This is why the fediverse is actually so fragmented instead of being one giant platform. Defederation was meant as a feature for blocking spammer instances but it is mainly used for censorship.

Yes you do, it's kind of a pain to setup the server software too. Misskey, pleroma, mastodon, they're all bad server side. Also it turns out the fediverse sucks ass, which is why I deleted my server.
